1601  US Youth Soccer Membership Form --  Note: This form requires a color (not black) wet-ink signature; a digital signature will not be accepted.  This form MUST be signed by the Parent/Legal Guardian OR a Player age 18 or over.  Players who turn 18 during the seasonal year must have a new form filled out when they turn 18. Players who are 18 MUST sign their own form.  Two copies of this form must be turned in.  You may make a copy of the signed form and turned the original and the copy in, or you may turn in two signed originals.  

1606  Team Roster Change in Status Form for Players & Adults -- this is a duplicate form, so an original with a color (not black) wet-ink signatures and a copy, or two signed originals, must be submitted.

1611  Guest Player Single Event Form -- original copy only required

1627  Foreign Document Translation Form -- original copy only required.  The translator who signs this form cannot be related to the player or the team.

 

1628 Adult Registration and Team Management Disclosure Form -- Adults must complete this form online.

•  Be sure to follow instruction on the CYSA landing page
•  Select "SASC" as the Organization, not Foothill
•  Be sure to include your email address and cell phone number on the completed form (you may have to write these in after the form prints)
•  You must check the box at the bottom of the online questionnaire form before you submit
•  You must submit an original with a color (not black) wet-ink signature and a photo copy
•  If you are registering with more than one team, you complete the online registration ONLY ONCE.  Make extra photocopies of your completed form; submit the original and one copy in one team packet; submit a photocopy, with a note stating the name of team that the original was submitted to, with all other team registrations.
