This form requires a color (not
black) wet-ink signature; a digital signature will not be accepted. This form
MUST be signed by the Parent/Legal Guardian OR a Player age 18 or over. Players
who turn 18 during the seasonal year must have a new form filled out when they
turn 18. Players who are 18 MUST sign their own form. Two copies of this form
must be turned in. You may make a copy of the signed form and turned the
original and the copy in, or you may turn in two signed originals. |

Adults must complete this form
online.
- Complete this form ONE TIME ONLY each
seasonal CYSA year; all teams you're associated with through the year are
covered via one online submission.
- Be sure to follow instruction on the CYSA
landing page.
- Click on the "I have agreed with..."
statement on the landing page to access the online registration form.
- Select "Sunnyvale Alliance SC" as the
Organization, not Foothill
- Please include your email address and cell
phone number on the completed form, even though these are not required
fields.
- You must check the box at the bottom of
the online questionnaire form before you submit.
- You must "sign" and "date" the online form
in the boxes provided by typing your name and date; this is an electronic
acceptance of all terms in the online document you have just completed.
- You no longer will receive a PDF copy to
print and sign; once you submit the online form you are done.
- Hold on to the confirmation number until
your team registration is completed.
