CYSA Forms that are frequently used:
 
1601  US Youth Soccer Membership Form  -- this is duplicate form, so an original with a "wet" blue ink signature and one copy, or two originals, must be submitted

1606  Team Roster Change in Status Form for Players & Adults -- this is a triplicate form, so an original with a "wet" blue ink signature  and two copies, or three originals, must be submitted

1611  Guest Player Single Event Form -- original copy only required

1627  Foreign Document Translation Form -- original copy only required
 
1628  Adult Registration and Team Management Disclosure Form -- this is duplicate form, so an original with a "wet" blue ink signature and one copy, or two originals, must be submitted
